More older adults now take medicines for opioid use disorder (OUD), such as methadone and buprenorphine, often for many years. Methadone carries known risks, including slowed breathing and heart-rhythm problems.

What they work on
- Measure how time to a medically significant fall relates to current and cumulative methadone dose among VA patients who started methadone for opioid use disorder between 2016 and 2025.
- Compare dose-associated fall risk for methadone versus buprenorphine, using target trial emulation to balance patient groups and Cox proportional hazards models to estimate risk.
- Assess how age group changes the risk of falls within each medication, with death from any cause and opioid overdose as secondary outcomes.
More about the research
These risks may grow as people age, take more medicines and develop other health conditions. Yet no large, high-quality studies guide doctors on safe dosing for older patients. This project asks how methadone dose, compared with buprenorphine, combines with age to affect the risk of serious falls and other harms.
Why it matters
Older patients taking methadone for opioid addiction may face growing risks of falls, overdose and death. Results on how dose and age affect these risks could help clinicians choose and adjust treatment more safely for aging patients. Results could also guide medication safety policy in the VA and beyond.
