Heart attacks and other cardiac events are the leading cause of death among firefighters, and many happen on duty. Short, broken sleep is a major risk factor for heart disease that can be changed.

What they work on
- Test whether the CBTi-based firefighter sleep health coaching program reduces sleep disturbance in 400 career firefighters across 20 departments, using a stepped-wedge cluster randomized trial with questionnaire and actigraphy outcomes.
- Measure effects of sleep coaching on cardiovascular risk markers, including blood pressure, heart rate, cholesterol and body mass index, along with sleep continuity and sleep-related impairment.
- Evaluate how the coaching program and its implementation strategies are adapted in fire departments, using a mixed-methods formative evaluation guided by the iPARIHS framework.
- Identify combinations of department-level factors that promote adoption of sleep coaching through a theory-based process evaluation embedded in the trial.
More about the research
Up to 40% of firefighters have sleep problems severe enough to screen positive for a sleep disorder. Cognitive behavioral therapy for insomnia (CBTi), a talk-based treatment that changes sleep habits and thoughts, works well in clinics. Whether a coaching version of it works in the workplace, especially in demanding jobs like firefighting, is largely unknown.
Why it matters
Firefighters face high rates of on-duty cardiac death and sleep disorders, and few can easily get proven sleep treatment. A workplace coaching program that improves their sleep could lower heart disease risk in a group vital to public safety. Findings on what helps departments adopt the program could guide sleep-focused workplace wellness programs nationwide.
