Doctors treating children with Crohn's disease often have to guess which medicine will work best. Crohn's disease is a type of inflammatory bowel disease (IBD) that affects over 50,000 U.S.

What they work on
- Compare the effectiveness of anti-TNF and anti-interleukin biologics in children with Crohn's disease, using causal inference methods in a validated multicenter retrospective cohort.
- Validate and extend these comparisons in a prospective observational cohort of children with Crohn's disease that tracks patient-reported outcomes.
- Pilot a pragmatic randomized trial comparing risankizumab and upadacitinib, two drugs not yet approved for children, in children whose Crohn's disease did not respond to anti-TNF therapy.
More about the research
children. When it is poorly controlled, it can cause pain, fatigue, growth failure and surgery. More than 10 advanced therapies are approved for adults, but only two, both anti-tumor necrosis factor (anti-TNF) biologics, are approved for children. Because of this, and because insurers often require anti-TNFs to fail first, it is hard to compare treatments fairly.
Why it matters
Children with Crohn's disease are usually treated with medicines chosen from adult data and small pediatric studies. Real-world comparisons of anti-TNF, anti-interleukin and newer off-label drugs could help pediatric gastroenterologists match each child to the therapy most likely to control disease. This could reduce pain, growth failure and surgery.
