Severe complications and deaths during and after childbirth in the United States have risen over recent decades. Hospitals use "safety bundles," standard protocols for obstetric emergencies, but no one knows how closely these protocols are followed or how much harm they prevent.

What they work on
- Measure how closely care for severe hypertension, obstetric hemorrhage, sepsis and abnormal maternal vital signs follows safety bundle recommendations, using EHR data from eight consortium hospitals.
- Estimate the risk of failure to rescue (death or permanent disability after a complication) when bundle guidelines are not followed, and identify the care decisions linked to it.
- Use EHR data to describe provider behavior and sort patients by risk, then run simulations that guide the design of future clinical trials and interventions.
More about the research
This project asks whether electronic health record (EHR) data can show this at scale.
Why it matters
Pregnant and postpartum patients facing hemorrhage, severe hypertension, sepsis or dangerously abnormal vital signs could benefit if hospitals can track in real time whether safety protocols are followed. EHR-based monitoring that works across many hospitals could help hospital leaders spot gaps in care and focus safety improvements where they are most likely to prevent deaths and lasting harm.
