One in three older adults in the United States dies with or from Alzheimer's disease and related dementias (ADRD). Hospice, the Medicare-covered program of comfort-focused care at the end of life, can improve care quality and lower costs for people with dementia.

What they work on

  1. Identify how Black, Latinx and Asian American care partners define high-quality end-of-life dementia care, through interviews with 90 people about hospice and other home-based supportive care.
  2. Document hospice organizational practices and policies that affect care for racial and ethnic minoritized people with ADRD, through case studies of 24 hospices using surveys, observation, document analysis and staff interviews.
  3. Develop a community-prioritized set of recommended services, clinical practices and policies for dementia hospice care, using six focus groups and a three-stage modified Delphi consensus process with care partners, clinicians, policymakers and researchers.

More about the research

Yet Black, Latinx and Asian American people with dementia are less likely to enroll, and standard hospice care is not designed for dementia-specific needs such as behavioral symptoms. This project asks how racism shapes access to hospice, its quality and how well it fits for racial and ethnic minoritized people with ADRD, and what should change.

Why it matters

Black, Latinx and Asian American families caring for someone with dementia face unequal access to good end-of-life care. The prioritized recommendations from this work could guide professional and ethics guidelines, hospice practices, and policy and regulatory changes. These changes could make hospice care fairer and better suited to people with dementia and their families.
