Teenagers who use alcohol or drugs face a higher risk of developing a substance use disorder (SUD) and of poor health later on. Yet few young people in the United States get treatment.

What they work on
- Adapt an evidence-based brief negotiated interview and change plan for youth at higher risk of a substance use disorder in the pediatric emergency department, using the ADAPT-ITT framework, a stakeholder board and youth pilot testing.
- Build an action plan and motivational-interviewing-based follow-up messages, such as appointment reminders, into the electronic health record through the Epic MyChart Care Companion app to support the move to outpatient care.
- Conduct a pilot randomized controlled trial comparing PEDS-TLC with standard care to measure feasibility, acceptability, linkage to outpatient care, and substance use at 30- and 90-day follow-up.
More about the research
Many of these youth already show up in emergency departments (EDs). That makes the ED visit a chance to connect them with ongoing care. However, little research shows how best to bridge that gap once they leave.
Why it matters
Few youth with risky substance use receive treatment, even though many pass through emergency departments. PEDS-TLC could give EDs a practical, EHR-integrated way to connect these adolescents to outpatient adolescent medicine or addiction care. Better linkage could, in turn, reduce substance use and its harms. Results from the pilot trial will indicate whether a larger trial is warranted.
