What they work on
- Identify barriers and facilitators to adding the 4Ms of age-friendly care to hospital-to-home transitions through interviews with older adults, caregivers, clinicians, administrators and community service professionals.
- Prioritize the identified barriers and facilitators using systems engineering risk-prioritization surveys that rate each on frequency, modifiability and importance.
- Co-design the adaptable ARRIVE-AT-HOME intervention through parallel five-stage design sessions with one team of older adults and caregivers and one of professionals.
More about the research
Existing programs for hospital-to-home transitions provide proven structures for safe handoffs. However, they lack key elements of age-friendly geriatric care. This project asks how to build those elements in.
Why it matters
Older adults leaving the hospital face high risks of readmission, falls, functional decline and death. An age-friendly transition program built with patients, caregivers and providers could address medication, mobility and cognition problems at discharge. The intervention is designed to scale to health systems and communities across the United States after testing.
