Many people with chronic kidney disease take a long list of medicines for several conditions at once, and problems with those medicines are common. One of the most important problems is a missing prescription.

What they work on
- Identify what would help nephrology clinics prescribe guideline-recommended heart- and kidney-protective drugs, such as SGLT2 inhibitors and statins, by interviewing clinic stakeholders using qualitative methods.
- Develop a pharmacist-led population health management intervention, which flags clinic patients missing recommended medications, using human-centered design with stakeholder input.
- Test the feasibility and acceptability of the pharmacist-led intervention for patients with chronic kidney disease in a pilot trial, to prepare for a future randomized clinical trial.
More about the research
Up to 30–40% of US adults with chronic kidney disease (CKD) are not on an ACE inhibitor or ARB (blood-pressure drugs that protect the kidneys) or on a statin. Newer medicines that protect both the heart and kidneys, such as SGLT2 inhibitors and GLP-1 receptor agonists, are also widely underused, and kidney specialists prescribe them especially rarely.
Why it matters
People with chronic kidney disease often miss medicines that are known to protect their heart and kidneys. A pharmacist-led program that finds these gaps in kidney clinics could help more patients get the recommended drugs. The results are meant to inform larger trials aimed at medication-related kidney and cardiovascular outcomes, and could lead to new models of CKD care.
