What they work on
- Evaluate whether Whole Person Care-Los Angeles, a social care program for Medicaid enrollees, improves diabetes screening, retention in care, medication adherence and blood sugar control compared with propensity score-matched non-enrollees.
- Measure the program's effect on diabetes complications, including cardiovascular disease, kidney disease, nerve damage and eye disease, plus emergency visits and hospital stays, using linked electronic health records.
- Compare program effects by sex, by race and ethnicity, and by their intersections, using a difference-in-differences design with input from a community advisory board.
More about the research
Part of the gap is linked to social hardships such as low income, unstable housing and not having enough food. People at the meeting point of these groups, such as non-Hispanic Black women, may face unique conditions that lead to worse outcomes. Programs that address social needs could help close these gaps, but little research shows whether they work for diabetes or for which groups.
Why it matters
Social care programs are expanding in safety-net health systems, but evidence on their effect on diabetes is limited. Findings could show whether such programs improve diabetes care and reduce complications among low-income Medicaid patients. They could also guide future programs that partner with community organizations to narrow differences in chronic disease outcomes by sex, race and ethnicity.

Measure how a multi-part food insecurity program changes clinical outcomes in Medicaid enrollees with diabetes, hypertension or depression at a federally qualified health center, using a difference-in-difference comparison with two propensity-weighted comparison groups.

Test whether REDDCAT2 delays hospital readmission within 90 days compared with usual discharge care for patients with type 2 diabetes, in a comparative effectiveness trial. The intervention pairs REDD-CAT social-needs screening with patient navigation.
