High blood pressure (hypertension) is a major driver of heart disease, and many adults who face barriers to care struggle to keep it under control. Remote patient monitoring (RPM), in which patients measure their blood pressure at home and share readings with their care team, could help.

What they work on

  1. Develop community health worker training modules for remote blood pressure monitoring, plus electronic health record support tools, using the Consolidated Framework for Implementation Research and user-centered design.
  2. Test whether remote monitoring supported by trained community health workers lowers blood pressure more than standard remote monitoring care among 300 patients with uncontrolled hypertension at Federally Qualified Health Centers, measured at 12 months.
  3. Evaluate adoption, acceptability, fidelity, cost and sustainability of the community health worker program using Proctor's Implementation Outcomes Framework, surveys and interviews with patients and clinic staff.

More about the research

But access to these digital tools and their use are uneven. Community health workers (CHWs), trained to help patients with social and practical barriers to care, could help close this gap. This project asks whether pairing RPM with specially trained, technology-enabled CHWs improves blood pressure control.

Why it matters

Patients with uncontrolled high blood pressure at community health centers could gain better control and reduce their risk of cardiovascular disease if trained community health workers help them use home blood pressure monitoring. The study could also show health systems how to add community health workers to primary care teams so that remote monitoring reaches more patients and is used over the long term.
