Colorectal cancer (CRC) is a leading cause of cancer death in the United States, and regular screening can catch it early. Yet only 44% of patients at federally qualified health centers (FQHCs) are up to date on screening.

What they work on

  1. Refine the eNav Toolkit, a web app for colorectal cancer screening decisions, reminders, bowel-prep instructions and education, through two rounds of field testing with 10 federally qualified health center (FQHC) patients each.
  2. Test in a multi-site randomized clinical trial of 400 FQHC patients whether the eNav Toolkit, compared with usual care, raises screening completion, screening quality and follow-up adherence at six months.
  3. Evaluate whether the eNav Toolkit works by changing perceived benefits, perceived barriers, perceived susceptibility and self-efficacy, the mechanisms proposed by the health belief model.

More about the research

FQHCs are community clinics that provide affordable care to underserved populations. Patient navigation, in which staff guide patients through the screening process, reliably raises screening rates across income levels and racial and ethnic groups, including at FQHCs. However, it needs staff time and money that low-resource clinics often lack.

Why it matters

A low-cost digital navigator could raise colorectal cancer screening among socioeconomically disadvantaged and underserved patients served by community health centers. These clinics often cannot afford human patient navigators. If the trial shows the eNav Toolkit works, the plan is to build it into electronic health records and offer it to FQHCs nationwide. The goal is to reduce the burden of colorectal cancer in these communities.
