After a stroke, many people move from the hospital into rehabilitation settings, and then between those settings. Post-acute rehabilitation (PAR) is the therapy and care that follows a hospital stay, and it is essential to recovery.

What they work on
- Identify subgroups of longitudinal post-acute rehabilitation use during the first year after stroke in Medicare beneficiaries, using machine learning clustering approaches that group people with similar care patterns.
- Characterize these rehabilitation-use subgroups by patient-, facility- and community-level factors, to understand who follows each care pathway after stroke.
- Determine how strongly patterns of post-acute rehabilitation use predict one-year outcomes after stroke, using Medicare and other nationally representative data.
More about the research
Yet many stroke survivors receive too little rehabilitation, or rehabilitation that is interrupted. This leads to poor continuity of care, wasted effort and worse outcomes. Past studies have looked at rehabilitation use only at single points in time. As a result, little is known about how people actually move through rehabilitation over the months after a stroke.
Why it matters
For stroke survivors, interrupted or inadequate rehabilitation can mean poorer recovery. By mapping how people actually use rehabilitation over the first year, this work could help health systems and policymakers improve continuity of care and get more value from rehabilitation. It could also identify which stroke survivors are at risk of gaps in care.
