People who use opioids face high risks of overdose and of HIV infection or poor HIV outcomes. Medications such as buprenorphine reduce overdose, drug use and infections, and can help people stay engaged with HIV prevention and treatment.

What they work on
- Design the Community-Based Opioid Treatment (CBOT) strategy and its implementation plan through interviews with patients, providers, nurses and other stakeholders, then pilot and refine it with 10 participants over three months.
- Compare the feasibility, acceptability and sustainability of CBOT, delivered by nurse care managers and outreach workers, with clinic-based opioid treatment in a randomized type-3 hybrid trial of 140 people who use opioids.
- Assess whether CBOT improves retention on buprenorphine, a medication for opioid use disorder, and HIV care outcomes such as viral load suppression or HIV testing uptake.
- Estimate the costs of delivering CBOT using continuously collected micro-costing data.
More about the research
Yet many patients drop out of care, and keeping them in treatment for a full year remains difficult even with good clinic-based programs. This project asks whether bringing care directly to people in their communities can keep them in treatment longer.
Why it matters
People who use opioids often fall out of addiction and HIV care, raising their risk of overdose and HIV. If nurse-led community delivery of buprenorphine and HIV services proves feasible, acceptable and affordable, clinics and health systems could adopt it to keep these high-risk patients in care longer, with potential benefits for overdose prevention and HIV viral suppression.
