Bariatric (weight-loss) surgery is the most effective treatment for severe obesity. Even so, many patients regain a large share of the weight over the following years.

What they work on

  1. Test whether topiramate, phentermine or their combination, added to lifestyle counseling, produces greater percent weight loss at 12 months than placebo in adults who regained weight after bariatric surgery, using a double-blind SMART trial.
  2. Determine whether the drugs reduce energy intake more than placebo, measured objectively with the Intake-Balance Method using doubly labeled water and DXA body-composition scans.
  3. Test whether drug treatment reduces the problem eating behaviors most common after bariatric surgery (grazing, loss-of-control eating and binge eating) compared with placebo.
  4. Compare treatment strategies for patients who respond poorly after 4 months, such as raising the dose of the same drug versus switching to phentermine/topiramate, while tracking safety measures such as heart rate, blood pressure and mood.

More about the research

Weight regain brings back weight-related illnesses, raises health care costs and lowers quality of life. Diet, exercise and behavior therapy have done little to reverse it. Medications might help, but no published randomized controlled trial has tested drugs for this problem, leaving doctors without solid evidence.

Why it matters

Many people who have bariatric surgery for severe obesity regain weight, along with weight-related health problems. This trial will provide high-quality evidence on whether widely available weight-loss medications can safely reverse that regain. Because its step-by-step design mirrors how doctors adjust treatment, the results could translate directly into clinical practice and improve long-term outcomes for post-surgery patients.

Carlos Grilo

Yale University, CT·Medicine & Clinical Research

Test whether behavioral weight loss, the medication naltrexone/bupropion, or both reduce loss-of-control eating and improve weight in 160 post-bariatric-surgery patients, using a 16-week double-blind, placebo-controlled 2×2 trial.

Rebecca Krukowski

University of Virginia, VA·Public Health & Epidemiology

Test the effect of five lifestyle components on 12-month weight loss in 608 adults taking second-generation anti-obesity medications, using a 2x2x2x2x2 factorial trial. The components are an education session, phone coaching, and digital diet, activity and weight self-monitoring.

Jason D Robinson

University of Texas MD Anderson Cancer Center, TX·Medicine & Clinical Research

Compare quit rates after six weeks of varenicline versus dual nicotine replacement therapy (patch plus lozenge) among 2,000 Texas smokers in a virtually delivered SMART trial, confirming abstinence by breath carbon monoxide.
